Care Coordinator Essential Criteria: Previous experience as a Care Coordinator for a busy Domiciliary Care Business. Local area knowledge across South East London areas. Familiar with the use of rostering software and ECM. Professional and responsive where there are issues and risks are reported by the staff teams and other health professionals Knowledge of local authority compliance and CQC requirements Champion of Service user rights Experienced in Mental Capacity Assessments, Care Planning and Risk Assessments Good communicator, well organised and proficient with Word,Excel and Outlook.
