In 2020 we opened a Discharge to Assess (D2A) nursing unit at Hayward Care Centre and are looking for a Head Nurse to be responsible for the day to day operational management of the care and nursing team, ensuring an excellent standard of care is set, delivered and maintained at all times. There is no ‘one’ model that will deliver discharge to assess. The main purpose of this unit is to support people who cannot go straight home after a hospital stay providing an assessment for longer-term care and support needs such as rehabilitation or reablement.   Hayward Care Centre, named after a local WW1 veteran, Reginald Hayward VC is located on the outskirts of the Wiltshire town of Devizes. The home offers compassionate, individualised residential, dementia, nursing and respite care for up to 80 residents.  The role of Head Nurse is pivotal within our homes. You'll be responsible for the day to day operational management of the care and nursing team, ensuring an excellent standard of care is set, delivered and maintained at all times, making critical decisions and leading on clinical issues within a person-centred care model. The role holds responsibility for the quality of nursing assessments, care planning, implementation and evaluation and associated care team training issues to ensure that at all times residents receive an excellent standard of care. We’re looking for a qualified Nurse with a current NMC pin combined with a minimum of 2 years experience leading a team of care staff in a residential care setting. Experienced in working autonomously, with the ability to understand and manage the complexities of the multitude of clinical and psychological factors that affect the health and well-being of our elderly residents.
