We have an exciting opportunity for an experienced carer, support worker, or team leader to join our Discharge to Assess (D2A) specialist team.
About the role:
You will be required to support with line management and the development of staff within the D2A team.
Support the ongoing service development and liaise with a range of health and social care professionals.
Assess individual’s needs and goals upon discharge from the hospital and write up the support plans following the initial visit and ensure person-centred care is provided.
You will be assisting with personal care such as personal hygiene, toileting, and dressing.
There will be a two week rota, with shifts between 7 am and 10 pm with alternate weekends.
You will be required to be on-call on a roster basis but will receive additional payment.
Previous line management experience is required as well as excellent communication and coaching skills.
Ability to understand and assess the needs of customers and create person-centred reablement plans.
Proactive, self-motivated and able to identify and implement solutions to ensure our customers regain their independence.
A full driving license and access to your own vehicle are required as you will need to be comfortable with lone working and travelling between customers.
£500 welcome bonus on successful completion of your probationary period.
Double time on bank holidays and an extra £1 per hour on weekends.
Mileage allowance of 45p per mile
Equivalent to 30 days of paid annual leave (inclusive of bank holidays), increasing to the equivalent of 33 after 5 years’ service (pro-rata)
