Job Description

We have an exciting opportunity for an experienced carer, support worker, or team leader to join our Discharge to Assess (D2A) specialist team. 

 

About the role:

 

  • You will be required to support with line management and the development of staff within the D2A team. 

  • Support the ongoing service development and liaise with a range of health and social care professionals. 

  • Assess individual’s needs and goals upon discharge from the hospital and write up the support plans following the initial visit and ensure person-centred care is provided. 

  • You will be assisting with personal care such as personal hygiene, toileting, and dressing. 

  • There will be a two week rota, with shifts between 7 am and 10 pm with alternate weekends. 

  • You will be required to be on-call on a roster basis but will receive additional payment. 

 

About you:

 

  • Previous line management experience is required as well as excellent communication and coaching skills.  

  • Ability to understand and assess the needs of customers and create person-centred reablement plans.   

  • Proactive, self-motivated and able to identify and implement solutions to ensure our customers regain their independence. 

  • A full driving license and access to your own vehicle are required as you will need to be comfortable with lone working and travelling between customers. 

 

Job Benefits:

 

  • £500 welcome bonus on successful completion of your probationary period. 

  • Double time on bank holidays and an extra £1 per hour on weekends.
