Job Description

We have an exciting opportunity for a Specialist Healthcare Support Worker to join our specialist Discharge to Assess (D2A) team.


About the role:


  • Our D2A service provides short-term, high-quality care and support to individuals who are well enough not to need an acute hospital bed.

  • Responsible for writing up the support plans following the initial visit and ensuring person-centred care is provided. 

  • Motivating customers and building confidence so our customers can achieve the goals they have set and return to independent living. 

  • You will be assisting with personal care such as personal hygiene, toileting, and dressing. 

  • Shifts will be a mixture of early and late shifts between 7 am and 10 pm and alternate weekends.

  • You will be required to be on-call on a roster basis but receive additional payment. 


About you:


  • Previous experience in caring for vulnerable adults with a wide range of complex needs is essential. 

  • Sensitive, caring, and passionate about delivering a first-class service to people within the local community. 

  • Proactive and self-motivated with great initiative and excellent communication skills. 

  • A full driving license and access to your own vehicle are required as you will need to be comfortable with lone working and travelling between customers. 

  • Respectful, friendly, and comfortable working in a team environment. 

  • You will hold (or be working towards) a QCF Level 3 in Health and Social Care. 

  • You can be located in the Sedgemoor, Burnham, and Cheddar areas as travel time is included in your contracted hours. 



Job Benefits:
