We are seeking a proactive Community Mental Health Practitioner to join our Intake team and provide front-line support for clients reaching out to Jami. This key role involves triaging referrals, signposting to external services, undertaking needs-led assessments, and delivering short-term interventions. This role will also support our step-down service, which offers lighter touch contact for clients transitioning out of active 1-1 support.
As a Community Mental Health Practitioner within the Intake service, you will be a key contact for individuals seeking advice and support, as well as the first point of contact for those interested in Jami services. You will conduct needs-led assessments for entry into Jami services, occasionally delivering short-term interventions where our 1-1 support is less appropriate. Working within our step-down service, you will provide light-touch monthly phone calls to those transitioning out of 1-1 support.
